Breast Augmentation (Implants) Raleigh, NC
Women choose breast augmentation for several reasons. While many feel their breasts are too small for their frame, or don’t reflect their natural femininity, others seek to balance uneven breasts or regain volume after weight loss or pregnancy. Both saline and silicone gel implants are available. It is possible to create a very natural, proportional look.
In some cases, combining augmentation with breast lift surgery not only increases volume, but provides a perkier, more youthful appearance.
The location of the incision can vary
- Periareolar – “around the nipple,” is made around the bottom of the areola.
- Inframammary – is made in the crease under the breast. Resulting scars can easily be hidden by undergarments and swimwear.
After making the incision, Dr. Diehl will create a pocket under or over the pectoralis muscle and insert the implant. We use Keller Funnels for all smooth gel implants to reduce trauma to the implant and possible implant contamination.
After the procedure, you will be bandaged and dressed in a special support bra. Wearing this garment for the first weeks after surgery will aid in healing. Discomfort is easily treated with pain medication.
After one week, most patients are able to return to work. After two to four weeks, you should be able to resume all other routine activities with the exception of lifting, pushing, or pulling more than 5 lbs. At the 4 week follow-up most patients are able to return to their usual exercise routine and activity levels. Scars will diminish after only a few months.
The vast majority of women who receive breast implants are extremely satisfied with their results. They report an increased sense of self confidence, and find shopping for clothes, and wearing them, far more pleasurable.
Breast Implant Exchanges and Release of Capsular Contracture
While breast implants can last for many, many years, situations do arise where a second procedure is advisable for cosmetic or medical reasons. Implants may rupture, an asymmetry may develop, or a capsular contracture (increased firmness) can also occur due to changes in the scar capsule that naturally forms around the implant.

Breast implant removal surgery, known as explantation, can be performed to change size and shape of breast implants, to remove the implants altogether, or to address a ruptured implant.
Breast implant removal is usually performed under general anesthesia, although occasionally an implant exchange or replacement for saline rupture can be done with IV sedation and local anesthesia. The explanation procedure can require an hour or more of surgical time, depending on the status of the implant capsule and whether or not the implant is ruptured. The procedure can be performed in an outpatient surgical center. The removal is made possible by an incision in the inframammary fold or the peri-areolar incision. Patients usually have drains for 3-8 days after the procedure.
